ENTRY REQUEST FORM

ALL WINNERS (or a representative) MUST be present at Carillon Park immediately following the parade to accept awards and have their picture taken with sponsors. Failure to be present (or have a representative present) will result in FORFEITURE of monetary prize. Also the Line-Up & Map will be downloadable the week of the event and emailed to the person of contact for each float!!!!
